Collective effort

In all areas of functions there are mismatch and misalignments
Resulting in delays and issues between steps.
Final  result will be reflected as a financial loss
But there are intangible losses and strains, which are the depletion of human value among members, in more understanding terms "demotivation"

The main cause of this is the deferentiation of functions and team work.
First part of the discussion is about concepts of team work.
The idea of team work is kind of collective effort which leads to success, where ecah member have a part to play, faliure to do team effort will result in  unsuccessful work. This is where te concept of team work fails, since it is very difficult to retain members performing related task/activity.
In persent sinario leader or someone should do extra effort just to maintain the team to work as a team. Some cases maintaining the team requires more resources and energy than actual taks itself.
To overcome such the ideology of team culture is introduced, situation is worse since to maintain that culture requires more and more resources resulting in division of responsibility in to functions,

Now this opens the second part of the discussion of different fuctions, main reason behind  all the complications is division of functions,
Ofcasuse one can argue that to streamline the system, functionally established divisions are required, true its one thought pocess, but those will complicate the system, any person can make a system complex but not possible to retain simple.
Coming back to main topic due to the wrongly chategorized functions other concepts will fail.
In order to succeed atleast within a main function all the members shall be directed on one task and one task only,
To elaborate, if manufacturing is the main function without unnecessarily creating sub functions one shall create a platform where everyone under that shall do only one thing, Manufacture the product being responsible for all applicable parameters.
Simple example is pulling a cart, only thing each one is supposed to do is "pull" make sure cart moves forward

Bottom line!  each individual shall be empowered to complete the required processing of input to desired output
